"Building and Connecting Communities"

Greater Manchester STP will use support from the Building Health Partnerships: Self-Care programme to: 

  • Build on existing activity around devolution and VCSE infrastructure to implement the Population Health Plan at a local and Greater Manchester level with a focus on joining up the range of person-centred, community approaches through the spread of what works, where and why.

The group has decided to focus their activity on Building communities in Heywood, Middleton, Rochdale (HMR) & Pennine – exploring the mechanisms (and incentives) to engage in self-care at a neighbourhood level.
